Worst Gym they say month to month and when you wanna cancel - they never cancel you.

I have been trying to cancel with no success. They told me they would send me the cancelation forms when i pay the outstanding amount and assist me with cancelling. it's now another month passed and still no cancelation. the amount outstanding keeps growing as they dont cancel or send the forms.
