Why Gym company is not the best

I have joined Gym company in Rand stadium South of JHB, in May 2013 As time went by the Gym went down it wasn't clean, the equipment was old. the service was pathetic!! Up until now I have not received a membership card and every time I went to the Gym I had to produce my ID number. I eventually sent them a mail in May 2014 telling them that I want to cancel the contract as I am not happy with the service nor the environment. I have not been contacted to date. Yesterday I have put in an Application for a car and I was denied as GYM COMPANY listed me on ITC without even contacting me, the claim they have sent me SMSes, they also have my email address as well as my postal address. they want me to pay R900 to clear my name. they have also Moved the GYM to Ormonde with out notifying all the users, PLEASE HELP ME
