unfairly Placed on ITC - Betty ruining my life

I signed a membership form with one of the consultants by the name of Moses in Randburg Square in 2014. Literally 2 days after signing the form I called the consultant (Moses) to inform him that I want to cancel my membership. Moses agreed and gave me Tapiwa's cell number whom I was to call to handle my cancellation. I called Tapiwa a few times the same day and the following day but I didn't get hold of him. Now 2 years later I check my credit status only to find out that a lady by the name of Betty Gumede has placed me on ITC right on my birthday stating that I owe Gym Company R8 428. I didn't receive any SMS, a call or an email prior to this to warn me that they were going to do this,. I've never been to any of their gyms before. I took my copy of the membership form of which in it I have written Moses' and Tapiwa's number to the meeting with Betty, she said I had no proof whosoever, she was unprofessional and threatened twice to walk out on our conversation. I spoke to Trevor (manager) he was of no help and told me that I have to pay R900 for them to rectify this, there is no mention of this amount in my contract. <br> Gym Company rectify this, if not I'm taking it to my Lawyers.
