Unfair ITC listing

Sometime in May 2012 i was at Maponya Mall doing my own things when i was approached by a consultant from the Gym Company asking me to join their join and i explained to him my reasons for declining which he said its not a problem but should i wish to join on a later stage i can fill out a form then he will call me. i went through the form and told him i was not working so i wont sign up for anything yet now in 2015 when trying to apply for better posts i am being told i am listed by Pauline Radebe, i have never set my foot inside that Gym nor do i have the membership card but i owe R8869 REALLY? I WONT PAY FOR SOMETHING I KNOW NOTHING OF, I NEED MY NAME TO BE CLEARED ASAP
