unaware blacklist

I think it was 2011 in Jabulani mall,I was approached by one of the agents from Gym company who was trying to persuade me to join and I told him I can't join since I travel alot. He then told me to fill up a form just for his records to show he spoke to me and I did..I suddenly was receiving calls from them that I need to pay my monthly fees which I fail to understand as I wasn't attending the gym.I phoned them and told them what occured and they must stop,now I've just found out that I'm blacklisted by them and I never received any communication about this,never even stepped in the door of their gym.I have recently paid off all my debts,starting on a clean slate, I don't need surprises like this one now. PLEASE COMMUNICATE VIA EMAIL SINCE I'M OUT OF THE COUNTRY TILL SUNDAY 20th December 2015
