REFUSING TO CANCEL MY MEMBERSHIP

I have been trying to cancel my membership now for 2months and your consultants are refusing to send a cancellation form. This month is supposed to be the last month that you debit my account. IF YOU DEBIT MY ACCOUNT ONE MORE TIME AFTER THIS MONTH................. CAN YOU SEND ME A DAM CANCELLATION FORM.
