No notification of default being listed

In 2012 I signed a contract with Winchester Hills fitness club, due to their facilities not being up to scratch e.g. , no water in showers and no proper gym equipment, I cancelled the contract within the cooling of period of 5 days. The manager and accounts lady was handed my cancellation letter and an affidavit from Mondeor police station. they debited my account 3 times and after querying it , they stopped the debit order and told me the contract was cancelled accordingly. Now 3 years later I find that they registered a default against my name in March 2015 under the Gym Company. I never even signed anything with The Gym Company. <br> I want this default removed from ITC/ Experian/ Transunion as :<br> 1. I cancelled the contract within the cooling off period due to poor facilities and Winchester Gym club not offering what they promised. <br> 2.I never received any notification in writing or telephonically from The Gym Company informing me of any debt or listing me as a default payment. <br> 3.I never received 20 days notification in writing from the creditor of the impending listing of this account on the credit bureau. <br> According to the law and the consumer protection act this is *******.
