Gym membership Cancelation not done by Gym Company Randburg Square

I took out a contact with Gym Company Randburg Square my contract expired in September and I advised them that I want to cancel it and asked Lerato Rampuane to please send me cancelation form on the 30 Sept, as she called me to pay the outstanding amount (as am not in town and cannot go in to cancel) she never bothered to send the form or direct me to the correct department. I then spoke to Antonator Mhlanga on the 11 October and she sent me the cancelation form, On the 28 October I paid outstanding balance of R598 as she advised and I sent back the forms on the 28 and she asked if I had paid and I sent her proof of payment on the same day but to this day my contract is still not canceled they keep on calling and asking me to pay when I ask about my cancelation they told me Antonator will get back to me and when I ask to speak to someone regarding my cancelation they transfer but nobody answers and it goes back to the operator that transfered the call. They have been taking my details saying they will call me back am still waiting since last month October. Today I get a call from Lerato Rampuane who could not transfer my call but told me to call back and ask for the manager when I asked to speak the manager regarding this cancelation and to my surprise I'm now owing R796 I don't know what for because the only month I did not pay is November as I thought my contract is canceled. I called back and asked to speak to someone regarding my cancelation as Lerato advised but again call could not go through they took my details and am waiting for their call back.
