Gym Company - I cannot believe how poor the customer service at the Newtown branch

I have tried a number of times to contact this branch to get a cancellation form since I'm no longer based in Gauteng and therefore cannot continue with the membership. Every time when I call it's a different excuse.

How hard can it be to share a cancellation form??? I'm so displeased with how this matter has been handled by the Newtown branch
