Clear my name

An agent from Gym Company forged my signature and wrong my surname wrong. I had told her that I'll join once I start working. I reported the matter to Gym Company because the agent blocked me then their response was that the agent is no longer working for them. They don't care about their service. They make money *****ulently. I want them to clear my name because I've never attended the gym and I don't gym at all.
