Forgery and Lies

Last month I submitted a claim for my Cellphone and the claim was declined because i was not using the cell number of the contract. i explained to the consultant (Bongani Khumalo MTN festival mall) by the time i was taking the phone that i will not be using the contract number, and he said there will not be a problem. My claim was declined and i requested for the policy Documentation. Goerge Thapedi sent me the documentation claiming I signed but all to find out they forged my signature and even the hand writing in the documents is not mine.

Now i need to know who signed those documents on my behalf between Bongani and Goerge.
