Injury on duty

I got hurt on work property during working hours. I slip on the wet floor and I damage my cartilage. A big chunk of bone marrow ripped lose inside the bone. I already had 2 surgeries. They never filled out a workers compensation form. And now they refuse to pay me compensation on my injury.
