GO HEALTH: Worst gym I've ever dealt with. Unable to cancel a contract or get in touch with anyone!!!

I have a 12 month contract with the gym in Fairlands. A month ago I completed an online form for cancelling the contract as I have relocated: https://www.gohealthclub.co.za/administration/. To date I have not received any feedback upon form submission, nor am I able to get a hold of either head office or the branch on any of the numbers provided on the website. My account is still being debited every month!!! This is ridiculous as they do not allow you to go to the branch and cancel your contract, nor do they allow you to do this telephonically. I even tried resubmitting the cancellation form that is provided on their site, however I'm unable to because the form was already submitted using my ID number and therefore cannot be sent through again. This is really frustrating. It should not be this hard to cancel a contract!!!!
