Unpaid PMB authorized claim for 2021

In 2021 I had to consult Psychologist. Funds were depleted we than app**** for PMB which was approved for 15 sessions. I haven't done all the 15 but it came to my dissatisfaction that GEMS is refusing to pay 3 sessions there is a letter provided as agreement which allows such consultations. I really do understand why we always have to fight with GEMS because if their poor service. Since I got injured nothing is going as per agreement I always have to fight before they do the right thing. This is so unacceptable, where is professionalism is this sector really ????
