Terminated Benificary without notification

On the 10 July at 1:05pm I submitted the certified copy of the affidavit A form requested by the medical aid. My dad falls and hurts himself badly on the 4th October and we rush him to the hospital he is admitted under the medial aid we go home at 2am the hospital calls saying my dad is not a benificary of the medical aid and we must take him out of the hospital. Firstly my mum answers the phone in a shock because we left my dad at the hospital. Then gets stressed out because we are questioned about the my dad been on the medical aid. He is in hospital and I told the hospital he is covered on a medical aid and I will get proof in the morning. It is now 3:02am I cannot sleep due to the shock of what I heard as every month I pay my medical aid without fail because I know my parents are I'll and that needs to be covered but hearing this is uncalled stress due to Ur misadmistration and you don't have the care in the world to tell me you have terminated my dads medical aid. I know things about it no sms or email to this regard. I demand my dad to be reinstated as he is in hospital at this moment and we cannot aford to cover this cash. I have all proof I have sent this document requested
