Gems refusing to change my option after giving me a form of motivation

Gems is refusing to change my option, I want to change my option cause gems is terminating one of my network hospital that is near where I am staying at the end of December 2022, and worse they didn't even notify us. I told them I cannot pay core of amount that I don't even have cause is still December I want to change option. The option change was closed on the 9th December and gems gave me motivation form to motivate and I did but still they decline, for the fact that they gave me motivate form to motivate which means is still possible to change, gems is useless they want to take our money but they don't wanna pay our hospital of choice instead they choose those useless hospitals without even doctors. I wanna change option I don't want this option.
