Complaint: Re-instatement of a dependent

I've been sending relevant documents for a dependent to be put back on the medical aid and the dependent needs chronic medication from the medical aid since April 2015 instead the medical aid has on many occasions stated that they haven't received my documents whereas I've been sending more documents countless times till now. Honestly I'm not satisfied with their services.
