Claim submission issues

I submitted a claim in July for a procedure which I had received authorization for. The claim was processed incorrectly and I have been trying to correct it since. I have contacted GEMS multiple times to rectify the issue and have resubmitted all the documentation that was requested and AGAIN it was rejected. I am so frustrated as I keep being told different things by different people and no one is able to resolve this issue.
