Worked but not listed

<p>I was employed at potchefstroom game from 2 ********** worked for 6 months even have the pay slip to prove it i resign and found a job at another company when I went to claim for my maternity money at the department of labor game was not visible so I went to ask for my UI19 to be told the system has been updated and they can't find me so the department of labor called the head office and requested them to email my UI19 and still today it hasn't been sent it's a year now this month so I want to know where am I going to find my UI19 and how come I was being deducted uif from my salary and still don't appear I want answers because according to the law the only time they can remove me it's after 5 year which I haven't reached so game management I would appreciate answers of what to do this is very unprofessional I wasn't volunteering I was employed and have a team number or was all that for show</p>
