Incompetence in taking down details

Game stores Pavilion. I opened a Game account and filled in the form with all of my details. I gave them my 3 month bank statement, a copy of my I.D. and all the supporting documents required. They took down all of my details wrong including my home address and my email address as well as my name!! I have not received one statement,I cannot log into my account to see my balances because my card number and email address do not match. I have not been debited for any charges as yet. How ***********, can people not copy details anymore.
