Provident fund contributions

Good morning

I have been working for a security company for 6 1/2 years. I resigned in the company, the problem now is that the former employer is refusing to sign my provident fund forms. Also on the statement I got from the provident fund offices I have learnt that there are lots of gaps where money would be deducted on my salary but never contributed towards provided fund. Please help
