UIF claim

I worked for this company for over a year as a sales consultant. I got retrenched last year July. I have been struggling to get the UI-19 form from my former employer to claim for UIF since last year. When I ask about it, their response is “I thought this was sorted out” the end of this month marks a year since I was retrenched. I still can’t claim for UIF because I don’t have the relevant documents. 2 of my other former colleagues also have the same problem. The one was also retrenched and the one was dismissed.
