IRP5 SUBMISSION UNDER MY TAX NUMBER

I have been an employee at Ford motor company during 2014 from 1st of June till the 1st of December where I resigned and joined PRASA METRO RAIL COMPANY. The following year 2015 Ford never removed me on their system especially that they started sending or submitting the tax return of their employee under my tax number, since then j have been writing disputes till today. I have approached Ford so many times and they still not fixing this mater and they don't want to give me a prove that this is not their matter so i have decided to take another step of reporting it. Please help
