Refusal to pay employees their provident fund

I was worked for the Gateway Food *****'s Market and they are refusing to payout the Provident Fund. They first deducted unknown monies from last pay and when queried there was no explanation and they just paid what was owed. Provident Fund Withdrawal Forms were completed on 14 June 2023 and received by Evelyn who is the HR. On 15 August 2023 she is confirming that she gave me incorrect forms, but no communication was sent to me regarding the issue.
