Fonesure is unreachable yet is taking premiums.

My cell phone is insured with fonesure. I have been trying to contact them for the last two weeks, and none of their numbers seem to exist, and I cannot get hold of them whatsoever. They have been debiting my bank account with an insurance premium for over two years, and now that my phone has been damaged, I cannot even speak to a representative, let alone claim. I have tried numerous times to cancel the contract, I have gone into the bank to stop them from taking of the monthly premium, and the bank says I have to contact the company directly in order to cancel the insurance contract... How on earth do I do that if the company is completely unreachable, they don't even seem to exist, yet they are taking money from me every month. Every number I have tried to call is either engaged, or does not exist.
