Unwilling to help

I am unemployed and went to FNB to get a UI2.8. Form done. The lady told me straight that I need to get the form from The Department of Labour. I have downloaded a copy off the internet. All they need to do is print it fill it in and e-mail it back to me. I started the process on 19 October 2024. Please help me now otherwise I will take this further.
