Unauthorised debit orders

Please explain to me how a debit order goes off my account without my authorisation and when I enquire about your consultants say they will send me a form, 3 weeks and still waiting for this stupid form. And now yet another debit order that I do not know of has gone off. You guys need to work on your security measures, now I must use my precious time to resolve your incompetence.
