Un-approved transactions

I have 2 transactions I’ve not approved in one month. The first one, a debit order I do not know with reference ‘mishnayokole’ when I try to reverse it, I cannot find it on my debit history.

The 2nd incident are 2 online purchases I did not approve. I received a notification (07/09/2021) for an online purchase while I did not purchase anything, within 5 seconds the purchase was already approved while I had not logged in nor approved it. 5 minutes later another online purchase notification came through, luckily I managed to decline and immediately changed passwords. I then tried to block my online profile and the system says unavailable. How are we to protect our accounts of the systems are unavailable. I would like to be refunded for these two transactions as I have not approved them.
