Skywise refund

As I'm sure you're aware, Skywise Airlines has been grounded for most of December. They are yet to resume operations. I bought an expensive R7999 unlimited ticket from Skywise on 27 October 2015, but have been unable to use it and have had to book alternate flights at huge expense to me.

Why am I telling this to you, my bank? The airline told me that FNB should reverse the transaction on their side as . it was paid for with my FNB Platinum card. To this end I enquired from not-so-Premier banking what the process is or how I go about it. A form was e-mailed to me with no explanation, that has no relation to what I asked. It's a dispute form with several boxes asking me to tick one to indicate in which way the transaction was **********. As such there is no way to correctly fill in the form as it wasn't. My reply to this went unanswered, and frankly not-so-Premier banking is very slow in responding in general (this is the second time I've had problems reaching and / or getting anything done through them).

Since HelloPeter seems to be the only way to get to people who actually can / want to help. Can someone please assist me?
