POOR DEBIT ORDER POLICIES IN PLACE

I made life insurance debit order disputes through my banking app. Out of almost 10 debit order disputes, only 3 or 4 were successful. Why would they approve only a few and not all?

I need clarity on this. Its either they approve all or none!!! Poor service this!!!
