please help fill a form

Please assist anyone I have exhausted all avenues I am very frustrated by my former employer FNB''s HR. All I need is a form filled from SASSA (Termination of employment form), if it's impossible to fill the form a letter with the letterhead and the information required by the form would be acceptable. I am not getting any response from the lady who is supposed to help me. I have logged multiple myq requests ,I have been to the branch all because of a form. It's over a month trying to get assistance. I felt so belittled when a lady from the HR called me and I wasn't given a chance to state my case the manner the lady spoke to me I felt very disrespected, her tone felt like I was being shouted at. She was telling me my request was directed to the incorrect department, I am just a former employee I don't know all the different departments within HR and their functions! I just need a form filled or a letter made out so I can get assistance, am losing hope and I know Fnb being such a reputable bank how it up hold's it's values that the service am getting is ridiculous.
