Please close the credit card account

I contacted the Premier Credit Card Call Centre to close my credit card account. I was sent a credit card closure form which I already completed, signed and sent back for the closure of the account. I emailed the form to *** and also sent it to *** but the account remains open. Can somebody please close this account before the next billing cycle as I do not want any issues?
