Incorrect captured by consultant

The first thing FNB did was to debit my 8year old daughters account for the amount of R120. This is for a replacement card, because the bank requires a birth certificate, which was incorrectly captured by the banks consultant. This is the 2nd time as my sons first card was also printed wrong.
