Former FNB employee, IRP5 request.

I am a former employee of FNB, I need my IRP5 from the time I was employed with them as these documents have been requested by SARS.

I tried logging in on the FNB HR MOBI. However, It returns an error message when I try to rest my password.

Please assist as soon as possible as I am now getting fined by the Tax man.
