FNB Magtape unpaid issue - Rates/Levies unpaid despite reflecting as sucessful

My wife and I have been to the bank several times due to all the issues experienced but my main issue is this - every month when she receives her salary she pays our rates and levies and utilities. This went off and says successful then later the month we get a notification saying it is unpaid and refers to Magtape unpaid. Now we are in a position where we are unsure if our lights and water is going to be shut off. This is ridiculous and the straw that is breaking the camels back. We are going to close this account with this bank Saturday and move to another bank. If my power is switched off we will be spamming social media and going in person AGAIN to make it clear that this is unacceptable. Furthermore previously when in arrears MULTIPLE penalties were app**** to her account instead of one. The teller said she would note this for a refund but nothing was done - GATVOL.
