FNB Business Bank Account

I have opened a business bank account and sent out all the necessary documentation, but I still receive emails saying that I have to send the following documentation A letter from the authorised representative confirming the names of those holding 25% or more of the Entity's voting rights at General Meetings

Is there a form of a way this letter needs to be written or some-kinda template to this
