failure to complete request to add a signatory

I had send out paperwork to request addition of another signatory onto our bank account on the 18th of March 2016 and to this date no one has made contact with me to finalise this. i sent this on the 18th and made several attempts to call the business desk to no avail each person putting me through and no-one completing the job but passing the buck. this is unprofessional and ***********.
