Extremely bad service

I have been requesting since last year for my personal banker to contact me and was told that she would contact me last week. I am yet to hear from her. I was told by the ex banker that he left last year and meanwhile sending all my details to him as nobody even bothered to contact me and let me know he left. All the while i thought he is ignoring me. I also requested to add someone for full signing rights last year and everytime I am told i will receive some forms that needs to be filled in. I keep receiving emails to say i will receive but that is it. No attachments on your mails. How difficult is it to send this form. I need someone to resolve these issues or send me a personal banker that i am paying for that is competent enough to dial my number as she does not know how to return the emails. I was also in the bank and advised that they cant do anything as they are also incapable of giving this form that needs to be sent and signed from above. Please contact me asap as this has been going on for almost half a year now.
