Deleted Mandate for signing powers

My Mother is disabled and has handed over POA to me for the past 15 years She signed a mandate around 2011 to give me access to her account.

FNB recently sent me a new bank card since the previous one was about to expire 3 months into using the card... the card is blocked/cancelled because apparently there is no mandate authorising me to use it.

How did the bank lose the original mandate? Why issue me with a new card if something is missing?

Now I have to take leave, make my disabled Mother go to a branch to do this all over again!!
