debit order authorisation form

I'm not happy about incompetence and the poor service i receive from Fnb home loan department especially when it comes to debit orders, i want to change a debit account from my account to another account Miso, Busisa sent me the wrong debit order authorization form something they did before, they're still doing it for the 3th time again, that process of filling the form, scan it, get my partner's signature and email it for me is really not convenient because i'm also at work and their system can't always inconvenient us .. im very angry about this
