Credit card closure

I have closed my credit card for over 4 months telephonically, the next thing two month later still appears on my credit record as still open amd active. I go into the branch, they tell me that i have to sighm some cancellation forms and which i was not told of before. Now we on fourth or fifth months and the account is still active and open in n my credit record. I need assistance regarding this. Its holding back my plans. I would really appreciate your response
