Charging me for services never received.

So, a month or so ago, I app**** for a credit card via your app. It gets approved and the card gets added to my profile but there is no available balance. A few days go past, and a lady calls me for additional documents, I tell the lady, do you know what, please close the application. A month goes pass, I go now into the app and you guys added fees on the credit card and you handed it over for collection lol. How do you charge people for something they never used or received. Please sort this out ASAP as I am going to log a complaint with the Ombud about this.
