Bank charges

Good day

Please can someone advise as to why there are two unpaid debit order fees for the same item? Also, please advise as to why I'm being charged for a replacement card when I have not received it yet? I also have no communication as to when the card will actually be delivered. I was notified that the card will be delivered on the 29th of January. Waited the entire day, no apology for not delivering but yet a replacement fee was already charged
