Account opened in incorrect name

In January I opened up a Savings account in my name. On the same day I opened up an FnbY account in my daughters name. FNB has opened both accounts in my daughters name, and I have only just found this out today, after transferring savings into that account each month. It has also affected my eBucks reward level, as I believed I was growing my reward level each month by transferring funds to my savings, but instead these funds are going to an account that is not even in my name. How did this error happen?
