Subject: Unauthorized Deduction and Cancellation Request for Policy LPS***

Dear [Ombudsman’s Office],

I am writing to raise a complaint regarding unauthorized deductions being made to my bank.Despite my explicit request for the cancellation of this policy, I have noticed that payments are still being deducted without my consent.

I did not provide permission for these deductions, and I have already asked First Loyalty Plus to cancel the policy. However, they have failed to act on my request, continuing to charge me without my authorization.

This ongoing issue is causing me significant frustration and financial inconvenience. I am seeking your urgent assistance to address this matter and ensure that the policy is cancelled immediately, with all further deductions halted and a full refund issued for the payments that have been taken since my cancellation request.My cancellation was within a month of purchase of this policy.

I would greatly appreciate your prompt intervention in this matter.
