Persistent debits post cancelation

Good day,

My father wrote you a notice of cancelation at beginning October. All of this was in vain as you still proceeded to debit his account in October, November and December! He made attempts to stop this dr with his Bank, that too did not help; the Dr still went off. Urgent request: reverse all the debits since his request was sent on the 6th of October
