Listed on ICT as a commitment on a non existent policy

Ffw has listed the premium that I was paying (R4500) as a commitment on my ICT records whereas I’ve cancelled the policy six months ago. When I call them to give me a letter showing that I’ve canceled my policy, they say they cannot find my name, they can find my husbands name. I was the one who was paying and they listed me on ICT.
