RE: CANCELLATION OF INSURANCE ON MY VODACOM BILLING

I, Ulf-Dieter Koepp, with I.D. ***, instruct the cancellation of premiums for the insurance for my smartphone. I do have an existing insurance policy with Santam; therefore, my smartphone is already insured with Santam.

I am attaching two documents for your reference/attention.

Please kindly amend the billing for next month cycle.

Please do take note that I am deaf.
