Unfair labour practice

I've been with the company for 6 years,and have achieved a lot,but the way employees are victimised there is unspeakable. May 2019 was involved in a car accident whilst on duty,no medical help offered and the accident was never reported to COIDA. I continued working with a dislocated shoulder,and was eventually admitted to public hospital where even the company refused to continue paying my salary because I was not on the road. I've lost chances to claim for RAF as I was refused help. I am living with sceptic arthritis at the moment, and recently been terminated for mistakenly driving into an open drain in the middle of the night in a poor lit area whilst on duty. Several grievances were brought to the managements attention,Hower they would rather victimise grieving employees than do the right thing. I have requested my UI 19 documents as I was terminated on the 23 July 2024,no help offered. Please help
