provident fund forms handed in and nothing done

I was an employee of Fidelity for approximately 13 months. I left fidelity and phoned the branch I was based at for the provident fund forms. They informed me I must go to the branch nearest to me. I went to the branch and collected the forms and handed them back in at Fidelity Klerksdorp branch. This was on the 28th of January. Since then I have phoned the head office in Johannesburg multiple times. They informed me they didn't receive the forms from the Klerksdorp branch.I phoned the Klerksdorp branch multiple times and they ask for my phone number and said they will be in contact with me. It is now May and I am still battling with this issue.
